In consideration of granting permission for my child, to participate in any activities organized by  Medina Basketball, I acknowledge, appreciate, and agree to the following terms:

I recognize the inherent risks of injury and illness, including communicable diseases such as MRSA, influenza, and COVID-19, associated with my child’s involvement in the programs. These risks include the potential for permanent disability and death. While specific rules, equipment, and personal discipline may mitigate these risks, they cannot eliminate them.

I, on behalf of myself, my spouse, and my child, willingly and knowingly assume all known and unknown risks, even those resulting from the negligence of the organization or other parties. I take full responsibility for my child’s participation.

I agree to adhere to the program’s stated terms and conditions for participation. If I notice any significant concerns regarding my child’s readiness or the program itself, I commit to promptly removing my child from participation and notifying the nearest official.

For myself, my spouse, my child, and on behalf of our heirs, assigns, personal representatives, and next of kin, I hereby release and hold harmless Medina Basketball, including its directors, officers, officials, agents, employees, volunteers, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of event premises. This release applies to any injury, illness, disability, death, loss, or damage to person or property arising from my child’s involvement or participation in the programs, whether due to the negligence of the Releasees or otherwise, to the fullest extent permitted by law.

Furthermore, I indemnify and hold harmless all the aforementioned Releasees from any liabilities incident to my involvement or participation in these programs, even if such liabilities arise from their negligence to the fullest extent permitted by law.

As the parent/guardian, I confirm that I have explained to my child/ward the risks associated with the activity, the responsibilities for adhering to rules and regulations and that my child/ward comprehends this agreement.

I, for myself, my spouse, and my child/ward, have read this Release of Liability and Assumption of Risk Agreement, fully understand its terms, acknowledge that we have relinquished substantial rights by signing it, and sign it freely and voluntarily without any inducement.
